That's a MD-11, not an L1011... #2's above the fuse, winglets, flap tracks extend below but not behind the wing, #1 and 3 are closer to the fuse than the Lockheed, the small horizontal stab means its an -11 and not a DC-10.

it`s called a 5th engine, or basically just a pod which is bolted to the wing with a temporary pylon, this normally occurs when an airline wants to transport a dead engine back to somewhere else as it is the easiest way to carry it.............

Even the old -200's looked gorgeous in this scheme. Beautiful aircraft and wonderful photo. Nice catch with the engine in transit as well. This practice fell out of favour with the more widespread use of freighter aircraft. A shame really, it would be quite cool to see a Korean 74 or something like that depart LHR with a 5th engine!
